The problems cited stem from the use of mark-sense optical scanning which,
to my mind, does not provide a satisfactory level of ease for voter
verification and is too complex and error-prone in the case of STV.

The only acceptable solution to my mind is OCR of hand-written digits.  
This is easy to achieve and fails-safe in all circumstances to the old
manual system.
